Enrolling or Withdrawing a Student

Withdraw A Student

Parent/Guardian must complete this form for withdrawal paperwork.

  • After initial contact, a withdrawal information will be emailed to parent/guardian.
  • A withdrawal packet will be provided to the new school once proof of enrollment is received.

Please ensure that all student books, materials and laptop/computer equipment has been properly returned to the NHS MAIN OFFICE.

Change of Address

If you have a change of address, you will need to provide a new proof of residency to the guidance secretary.

New proof of residency can be one of the following:

  1. Deed/mortgage statement.

  2. Full, SIGNED (by renter and landlord), notarized Lease with two pieces of mail (bills, statements, voter information).

  3. If you are living with a relative or other person and do not have a rental agreement, ONLY our Pupil Personnel Worker may accept other forms or information to validate a proof of residency. Contact the NHS Pupil Personnel Worker via email.
